Position Description

Introduction:

                
In all companies it is important to have clear lines of responsibility. For this purpose, all new employees sign a contract when hired. Below is a position description I have revised for my current company Caring Hands Home Care. While revising this piece of our policy several key elements of composition were considered. First was the content of the position description. The content had to cover all of the duties of an LPN  without becoming overwhelming to the reader. Separating the information into sections  helped to break the information down allowing the reader to digest and understand the information. Another element to this piece was word agreement. Each line in the Performance Responsibilities and Standards begins in a present tense verb. Another element of this piece is the format of the document. Since the position description is meant to be a printed document it has been adapted to be posted online. The original document includes a header that gives the company's information. The section Performance Responsibilities and Standards is formatted in two columns instead of the singular format depicted here. 


Position Title: Home Health Care Licensed Practical Nurse

Position Scope:

-          To ensure quality and safe delivery of Caring Hands Home Care (CHHC) services.
-          To assist in the provision of CHHC services that reflects the agency’s philosophy and standards of home health and nursing care of assigned clients.

Position Qualifications:

                Required:               -Graduate of an accredited school of nursing.
- Licensed as a practical nurse
-Registered with the New York State Board of Nursing.
Preferred:               -Experience in home health care or related health care field.

Physical Requirements:

-          Visual/auditory skills for communication and observation.
-          Ability to perform tasks involving physical activity, which may include heavy lifting and extensive  bending and standing.
-          Ability to effectively deal with stress.

Performance Responsibilities and Standards:

1. Ensures quality and safe delivery of CHHC services.
 a.        Participates in development and implementation of client plans of care as per CHHC policy and procedure.
b.        Participates in client case conferences according CHHC policy and procedure.
c.        Provides nursing care that reflects client care plans.
d.        Submits information regarding client care plans to the CHHC Registered Nurse in a timely manner.

2. Implements current nursing practice.
a.        Receives client assignments and reports from the CHHC Registered Nurse.
b.        Discusses client care plans are with a CHHC Registered Nurse on a regular basis.
c.        Implements nursing interventions for client care plans.
d.        Accurately reports observations of client condition to the CHHC Registered Nurse.
e.        Documents client clinical records per CHHC policy and procedure.
f.         Implements infection control and safety policies and procedures as per CHHC policy and procedure.
g.        Implements clinical care as per CHHC policy and procedures.

3. Cares for clients holistically.
a.        Communicates identified client needs to the CHHC Registered Nurse in a timely manner.
b.        Responds to client requests promptly.
c.        Maintains Clients’ overall appearance to reflect good personal hygiene.
d.        Maintains a neat and orderly client environment.
e.        Assists Clients' to meet socialization needs.


4. Promotes effective written/verbal communication.
a.        Demonstrates skill in conflict resolution.
b.        Promotes a positive work environment for CHHC members.
c.        Participates in CHHC staff meetings.
d.        Participates in CHHC events.
e.        Understands information regarding changes in CHHC operations and policies and procedures.
f.         Clarifies uncertainties with CHHC staff members in a timely manner.
g.        Documents information regarding interpretation of CHHC philosophy, objectives, and long-range plan is demonstrated as necessary.
h.        Maintains and respects Consistent and concise lines of authority and responsibility.
i.         Cooperates with CHHC staff members and the CHHC Director of Nursing pursuant to CHHC philosophy and objectives.
j.         Initiates consultation with CHHC RN or CHHC Director of Nursing as issues and concerns require.

5. Gives accurate information to clients and client families   
     regarding costs of CHHC services.
a.        Consults with CHHC registered Nurse regarding financial matters.
b.        Refers clients and client families to appropriate individuals/agencies for assistance with financial concerns.

6. Provides information regarding equipment and supply
     needs of CHHC
a.        Submits Information regarding equipment and supply needs of CHHC to the CHHC Director of Nursing in a timely manner.
b.        Participates in the implementation of CHHC budget plans to ensure operation within approved limits.

7. Uses equipment and supplies effectively and efficiently.
a.        Cost effective and accurate utilization of supplies and equipment is demonstrated.
b.        Communicates information regarding malfunctioning equipment and inadequate supplies to appropriate individuals.
c.        Communicates information about need/reordering of supplies and equipment to the appropriate individuals.
d.        Discusses information with clients and client families regarding effective and efficient use of supplies and equipment.

8. Participates in payroll programs following CHHC policy.
a.        Accurately documents time cards.
b.        Complies with payroll schedule.
c.         Ensures time card documentation is signed by authorized individuals.

9. Complies with CHHC policy and procedure.
a.        Maintains Current professional licenses of a CHHC Licensed Practical Nurses
b.        Submits a yearly physical and PPD to CHHC to ensure health and safety of clients and staff.
c.        Demonstrates job performance of CHHC Licensed Practical Nurse in compliance with CHHC policy and procedure.
d.        Communicates need for change in CHHC administrative and clinical policy and procedure to the CHHC registered nurse or CHHC’s Director of Nursing. 
e.        Participates in implementing systems of evaluating in CHHC delivery of services.
f.         Participates in development and implementation of emergency procedures.
g.        Maintains confidentiality of client information is demonstrated per CHHC policy and procedure

10. Complies with federal, state, and local rules and
       regulation.
a.        Consults with CHHC’s Director of Nursing regarding federal, state, and local rules and regulations.
b.        Participates in implementation of a client complaint system is demonstrated.
c.        Completes and submits regulatory forms in a timely manner.

11.  Participates in personal growth and development.
a.        Develops and implements professional career objectives.
b.        Demonstrates current knowledge regarding professional development in the delivery of healthcare services.
c.        Consistently participates in education programs.
d.        Reviews of current literature regarding healthcare.

12. Promotes education for client, client family, and
       community education.
a.        Identifies client, client family, and community education needs.
b.        Participates in education programs to address identified client, client family, and community education.

13. Promotes education for CHHC staff members
a.        Reports any identified needs of staff members to CHHC Registered Nurse or CHHC’s Director of Nursing.
b.        Aids other staff members in their own personal career development.
c.        Notifies other staff members of upcoming educational program
